What to expect at an office visit
As a new patient, how will my visit be different from an established patient?
We are glad you asked. Since we are both starting at ground zero, we need a little more time to enter your information. We like to know which pharmacy you like to use, what allergies you have, what meds you are on and when you take them, etc. Once that is all in our EMR, we are focused and ready to move forward. The visit will look similar to an established patient visit, except with more questions to get your history and time to get to know Dr. Slatosky.
What should I expect at my first visit?
At your first visit, you will meet our first friendly face at the front desk to complete our new patient paperwork. We ask that all new patients arrive 15 minutes early. Also, bring your insurance card, ID, and a list of your medications. One of our nursing assistants or certified nursing assistants will take you back to one of our exam rooms where we will take your temperature, get blood pressure, and update your current medications. on the way to your room, you’ll stop by and get your height and weight.
How much time would I have with Dr. Slatosky?
Keep in mind, we NEVER double book our appointments. Your time is your time, so make sure you tell us everything you need addressed before you are scheduled so we can provide the time you need and deserve. For example, if you need to be seen for a cold, we would schedule you for 15 minutes with Dr Slatosky. If you need something else addressed, in addition to the reason for your visit, depending on the schedule for that day, there may not be adequate time to address both concerns in the same visit. That's why we advise you to let us know what you would like to be seen for when you schedule your appointment so that we don't run out of your time.

Appointments
How can I make an appointment?

What type of services do you provide? We offer comprehensive care for ages 2+ and all the way up. This includes annual physicals, acute illness care, weight loss/management, and chronic disease management such as high blood pressure, diabetes, and asthma.
How soon could I get an appointment once established with your practice?
Our mornings fill up quickly for patients who are fasting. We are currently scheduling those appointments 3-4 weeks out; however, we do have a wait list for cancellations. If you can schedule in the afternoons, we can usually get patients in same day or within 1-2 days. We always try to keep a few appointments open daily for urgent needs and you may be able to skip the urgent care wait line by calling us to see if we have availability.
What if something else comes up other than what I am scheduled for?
Please call us before your appointment so that we can make sure we can address all of your concerns within the scheduled amount of time we have set aside for you.
What should I bring to my appointment?
If you have a commercial insurance, Third Party Administrator, Medicare, or supplemental policy, the patient is required to present their insurance card at every visit, even if there are no changes. Medication lists are very helpful and highly recommended.
Are walk-in appointments available?
Walk-in appointments are not accepted. Please call the office to schedule. We try to schedule established patients as soon as possible.
Are telehealth visits available?
We do not offer telehealth visits.

Prescriptions
How do I request a prescription refill?
Contact your pharmacy directly or call the office. Please allow 48 hours for processing.
Can I get a same day prescription refill?
We try to process refills quickly, but same day service is not guaranteed. Please plan ahead to avoid delays.

Lab Work and Specialists
How do you collect labs?
We draw samples in office to help make things more convenient. We collect urine, fecal, blood, and other tests ordered, and the collecting laboratory pick up the samples at the end of every day. We also offer lab testing in office for microalbumin/creatinine, flu, strep, covid, and blood glucose.
When will I receive my lab results?
Pretty quickly. We typically share results by phone. Some patients request that we only call if something is abnormal. We are flexible. We've heard many times that patients get their results from the lab company before we do.
Can I pick the specialists I want to see?
We want to know what you want to know, so we are happy to refer you to specialists from Asheboro, High point, Winston-Salem, Burlington, and Greensboro. Really, if you have a preference, just let us know during your visit.

Cancellations
What if I need to cancel my appointment?
As much as we love to care for you, some patients need to cancel their appointment last minute. We understand that unforeseen things happen. If possible, please give a 24 hours’ notice if you are unable to make your appointment or need to reschedule by calling our office.

Insurance, Payment & Billing, Medical Records
What insurance plans are accepted?
We are contracted and in-network with the majority of insurance carriers. However, there are a few we are not, so call us to verify that your insurance plan is accepted.
What if I don’t have insurance?
Self-pay patients are always welcome and preferred because we like it when it's all about you. We offer exceptional self-pay rates and will provide you an estimate of your vist cost before you are seen. All fees must be paid on the date of service to receive the self-pay discount. For patients who are partners with med shares, we will make sure you have everything you need before you leave to file your claims.
What forms of payment are accepted?
We accept cash, check, and credit card (Visa, Mastercard, Discover)
When is payment due?
Copays and out of pocket costs are due at the time of service.
How do I make a payment?
You can make payments over the phone, in the office, or by mail.How do I transfer my medical records to Pine Haven Family Medicine?You may complete an “authorization to release health information” form which allows us to request your medical records.
